Direct or tubed flaps involve having the flap connected to both the donor and recipient sites simultaneously, forming a bridge. This allows blood to be supplied by the donor site while a new blood supply from the recipient site is formed. Once this happens, the bridge can be disconnected from the donor site if necessary, completing the transfer. A free flap has the blood supply cut and then reattached microsurgically to a new blood supply at the recipient site.
=== Mechanism of action === Zalcitabine is an analog of pyrimidine. It is a derivative of the naturally existing deoxycytidine, made by replacing the hydroxyl group in position 3' with a hydrogen. It is phosphorylated in T cells and other HIV target cells into its active triphosphate form, ddCTP. This active metabolite works as a substrate for HIV reverse transcriptase, and also by incorporation into the viral DNA, hence terminating the chain elongation due to the missing hydroxyl group. Apart from inhibiting retroviral reverse transcriptase, ddC inhibits mitochondrial DNA polymerase gamma, resulting in a dose-limiting toxicity.

=== Signs and symptoms === The classic clinical syndrome for vitamin B6 deficiency is a seborrheic dermatitis-like eruption, atrophic glossitis with ulceration, angular cheilitis, conjunctivitis, intertrigo, abnormal electroencephalograms, microcytic anemia (due to impaired heme synthesis), and neurological symptoms of somnolence, confusion, depression, and neuropathy (due to impaired sphingosine synthesis). In infants, a deficiency in vitamin B6 can lead to irritability, abnormally acute hearing, and convulsive seizures. Less severe cases present with metabolic disease associated with insufficient activity of the coenzyme pyridoxal 5' phosphate (PLP). The most prominent of the lesions is due to impaired tryptophan–niacin conversion. This can be detected based on urinary excretion of xanthurenic acid after an oral tryptophan load. Vitamin B6 deficiency can also result in impaired transsulfuration of methionine to cysteine. The PLP-dependent transaminases and glycogen phosphorylase provide the vitamin with its role in gluconeogenesis, so deprivation of vitamin B6 results in impaired glucose tolerance.

Single-cell proteins (SCP) or microbial proteins refer to edible unicellular microorganisms. The biomass or protein extract from pure or mixed cultures of algae, yeast, filamentous fungi or bacteria may be used as an ingredient or a substitute for protein-rich foods, and is suitable for human consumption or as animal feeds. Industrial agriculture is marked by a high water footprint, high land use, biodiversity destruction, general environmental degradation and contributes to climate change by emission of a third of all greenhouse gases; production of SCP does not necessarily exhibit any of these serious drawbacks. As of today, SCP is commonly grown on agricultural waste products, and as such inherits the ecological footprint and water footprint of industrial agriculture. However, SCP may also be produced entirely independent of agricultural waste products through autotrophic growth. Thanks to the high diversity of microbial metabolism, autotrophic SCP provides several different modes of growth, versatile options of nutrients recycling, and a substantially increased efficiency compared to crops. A 2021 publication showed that photovoltaic-driven microbial protein production could use 10 times less land for an equivalent amount of protein compared to soybean cultivation. With the world population reaching 9 billion by 2050, there is strong evidence that agriculture will not be able to meet demand and that there is serious risk of food shortage.
=== Dopaminergic control of the pars intermedia === The pituitary gland consists of three parts: the pars nervosa, the pars intermedia, and the pars distalis. The most critical structure to PPID, the pars intermedia, is regulated by the hypothalamus. The neurons of the hypothalamus innervate cells known as melanotropes within the pars intermedia, releasing dopamine which then binds to dopamine receptors on the melanotropes. The activation of these dopamine receptors leads to the inhibition of proopiomelanocortin (POMC) production in these cells. In PPID-affected horses, dopamine is not produced from these neurons, leading to dopamine levels about 10% of the level normally found in the pars intermedia. This is thought to be due to neurodegeneration of the neurons, secondary to free radical formation and oxidative stress. Without regulation from dopamine, the pars intermedia develops hyperplasia and adenoma formation, leading to gross enlargement and excessive production of POMC. These adenomas also have the potential to compress the hypothalamus and optic chiasm.
